Sports Physicals

2012-13 Physical forms are available here or at the high school office.  Complete your information on the form before going to your appointment, a parent must sign it before the physical will be given. Remember you will be required to give a urine specimen at the time of the appointment.  

YOU WILL NOT BE ALLOWED TO PARTICIPATE IN ANY SPORTS ACTIVITY OR PRACTICE UNTIL YOU HAVE RETURNED THE COMPLETED PHYSICAL FORM TO THE SCHOOL

The elementary, junior high and high school use the same computer accounting system for meals. Parents may deposit any amount into the family account. Parents may send one check for all their children's meals. Upon reaching a $2.00 per student balance, parents will receive a daily automatic text or email message as a courtesy reminder. 
Students in grades 7-12 may select a la carte food items for an additional charge to their lunch account, unless their account has a negative balance.  Parents are responsible for talking with their children if they do not want them to select these items.
Free and reduced price application form: Read these instructions first, the applications are also available or at the high school school office. You will need a current version of adobe reader installed on your computer to view these forms.  The form can be completed online, but you will need to print it out, sign it and return it to the school office.
Textbook Fees:  Grades K-6: $35 per student Grades 7-12: $45 per student with a maximum of $125 per family.
Fee waiver applications are available here or at the school offices.

In addition, those students receiving a school-issued laptop may join an insurance group for $30 which will cover damages up to $300.  If you decide not to purchase the insurance, the student is responsible for any damages or repairs up to the full cost of the laptop.  (A broken screen currently runs $175.00)

Athletic Fees  
FM Students are admitted to home athletic events free of charge, but children must be accompanied by an adult.   Adults may purchase an All Activity Pass for the 2011-12 school year for $100.00.  Admission fees to individual events are as follows:  Sinlge Varisty Game: $5.00; Single JV Game $3.00;  Single JH Game: $3.00

Preschool Tuition Fees for three and four year olds:

2 days/week: $50 month - 4days/week: $100 month -  you may be eligible for a reduced rate - please check with Mrs. Forney

First Day of Classes - August 15, 2012

Fremont-Mills Community Schools will open the 2012-13 school year Wednesday, August 15th. Classes will run a normal full-day schedule with busses running at normal times.
Questions regarding bus routes, times, and schedules can be directed to the School Transportation Department at 712-629-2325 ext 404.
Parents are encouraged to listen to KMA 960 AM; WOW 94.1 FM or 590 AM for weather early dismissals.
